You will need the following items or documents for application: Palm Beach Staffing Home Health Caregiver Application Information.

	Current Driver's License (if no license, Florida picture ID)
	Social Security Card
	Current Auto Insurance Card
	If born outside of the USA, Current Resident Card, Authorization to work or proof of citizenship (Voter's ID, Passport, Naturalization paper)
	Health certificate (less than 6 months old) from a MD or DO that states caregiver "appears free of communicable diseases, including TB". Health certificates from Chiropractors will not be accepted.
	RN, LPN or RRT license; HHA or CNA certificate
	Current CPR card
	AIDS/HIV continuing education certificate
	Prevention of Medication Errors (RN or LPN only)

Domestic Violence and 2 hour Alzheimer's training (2 hour training with State approved curriculum) will be required within 30 days of start of work.

At your appointment be prepared to complete the items listed below

HHA or CNA with HHA certificate	Pre-application test, approximately 20 questions (multiple choice, matching and essay) covering basic HHA knowledge
CNA (without HHA Certificate)	HHA Competency Test, 104 questions (multiple choice and true/false) and skills assessment
RN/LPN	Medication test, approximately 60 questions (matching and multiple choice)
All Applicants	Biomedical Waste Handling test. You will be given an information sheet on handling Biomedical Waste in a home environment and a test with 10 questions from material in the information sheet.

Historically, Palm Beach Staffing Home Health has low turnover in clerical and nurse supervisory staff positions but occasionally there are openings.
